NBA Game Trip Consent Forms

The Sabaoth Group Inc. as part of the Nets Athletes Assist program is sponsoring future trips to the Nets home games. For more information about this program please see our previous news article.

In order to attend all children must have a signed Consent form and Agreements of Discipline form, both available at the Sabaoth Group office and below.

All forms should be submitted to the Sabaoth Group office before the date of the trip. For more information please contact the Sabaoth Group office.
